National Nursing Week

National Nursing Week is May 8-14, 2023. The theme for this year is Our Nurses. Our Future. This theme captures the diversity of roles and contributions of nurses in the healthcare system.

The Rapid Response Nursing Team of the Winnipeg Regional Health Authority provides short-term services to support individuals in managing their healthcare needs. The nurse and the individual work collaboratively to develop a plan of care that is individualized and based on their health care goals. The goal is to support the individual to receive the right care, in the right place, at the right time.

The Manitoba Acute Coronary Syndrome Network collaborated with the rapid response nursing team to provide a new patient-centered model of care called ‘tailoring post discharge’, starting in February 2023. This novel model of care was developed to assist individuals in safely transitioning from hospital to community after a heart attack. The rapid response nurses see the individual in their own home, to monitor their recovery and provide in-depth education and support within 24-48 hours post-hospital discharge. The rapid response nurse and patient mutually develop learning needs and goals that are important to the individual in their heart health recovery. In addition, nursing assessment and collaboration with other healthcare providers ensure the individual is well resourced and connected.

The rapid response nursing team is a collection of highly-trained and educated healthcare professionals that are integral to supporting safe transitions from hospital to home. We thank them for their service and dedication.
