TPD-REACH: Transforming Post-Discharge Care for ACS Patients Across Manitoba

The TPD-REACH study (Tailoring Post Discharge - Remote Education and Access for Cardiac Health) is now enrolling at St. Boniface Hospital and aims to improve outcomes for patients following acute coronary syndrome (ACS). Despite advances in in-hospital care, up to 26% of ACS patients in Manitoba return to the emergency department or urgent care within 30 days of discharge - often for concerns that could be addressed with better support and follow-up. TPD-REACH evaluates whether a short-term, digital health–based program can reduce these unexpected healthcare visits by delivering structured post-discharge education alone or in combination with additional clinical support. The intervention is delivered through a secure remote home monitoring platform, providing patients with daily education modules, and depending on the study arm, symptom tracking, access to virtual cardiologist follow-up, and real-time support.

Building on the success of the ACS-24 pilot program, which demonstrated fewer emergency visits, high patient satisfaction, and safe early discharge of low-risk ACS patients in Winnipeg, TPD-REACH expands this work to include low- to intermediate-risk patients across Manitoba, including those in rural and remote communities. The study will enroll 350 participants and examine clinical outcomes, psychosocial well-being, patient engagement, and cost-effectiveness at 30 days post-discharge. By integrating care into patients’ homes through digital tools, this work aims to address longstanding gaps in post-discharge support, reduce strain on emergency services, and improve recovery experiences. Findings from TPD-REACH will help inform scalable, equitable models of post-ACS care across Manitoba’s diverse healthcare settings.
