Tailoring Post Discharge: Enhancing Digital Understanding of Cardiac Care and Transition Education (TPD-EDUCATE)
Every year, over 2,100 Manitobans are hospitalized with a heart attack. Discharge from hospital is a vulnerable time where many patients can leave without fully understanding their condition, medications, or recovery steps. In a recent survey, 1 in 4 patients felt overwhelmed by the amount of information they received before discharge and nearly 1 in 5 left with unclear information, highlighting major gaps in patient education.
TPD-EDUCATE is a Manitoba ACS Network initiative to transform heart attack education through a personalized, interactive digital platform. By replacing one-time paper handouts with videos, interactive activities, and patient-centered modules, the platform ensures patients and caregivers receive clear, engaging, and personalized information - both before discharge and at home. This innovation has the potential to improve recovery, reduce complications, and bridge rural-urban health gaps.
What’s Next
Our team is conducting patient and caregiver focus groups to identify education needs and co-design the platform. In partnership with healthcare providers, we will then develop and refine the digital modules, ensuring they are accurate, relevant, and scalable across Manitoba hospitals, clinics, MyHealth teams and cardiac rehabilitation programs.
