A new project: Tailoring Post Discharge (TPD) (ACS-24)
In Manitoba, over 3,000 people have heart attacks (also called ACS, or acute coronary syndrome) every year. Patients with ACS need a tailored discharge process including unique education and healthcare follow-up for their post-ACS journey.
Research by our network demonstrates that low-risk ACS patients discharged from St. Boniface Hospital (SBH) within 48 hours (presumed low-risk) had a 30-day emergency room (ER) visit and hospital readmission rates of 22% and 4%, respectively. ER visits and re-admission leads to increased patient anxiety, delays in return to work, and is associated with poor patient outcomes. ER visits are also associated with a 50% increase in cumulative hospital costs, leading to an added burden on the healthcare system. Overall, we found that the immediate outpatient care of ACS patients needs to be improved. During a recent survey of 42 ACS respondents, 7% felt the information they received about their heart attack was not clear, or was at a level they could not understand, and 24% felt it would be hard for them to remember the information they had received.
We’re launching a new project to fit this need - Tailoring Post Discharge (TPD), referred to as ACS-24, a quality improvement and research project aimed to help patients receive the ongoing medical care, education, and support based on their needs, wants, and level of understanding. This new model of care will allow patients and their loved ones to access and receive ongoing care that fits them and their needs, and supports them in their recovery at home.
